I have updated my resources page on the Terminally Ill Adults (End of Life Bill), but particularly welcome any position statements and the like from relevant organisations that I might have missed.
As highlighted on that page, the Public Bill Committee has set out a call for evidence, with a deadline of “as soon as possible.” The Committee has given some guidance as to format and the like, but also as to what such written evidence should cover:
Your submission should address matters contained within the Bill and concentrate on issues where you have expertise and on factual information of which you would like the Committee to be aware.
Your submission could most usefully:

- highlight or discuss views on or concerns with the existing provisions of the Bill;
- suggest amendments to the Bill, with supporting explanation; and
- (when amendments are published) support or oppose amendments tabled to the Bill by Members of Parliament, with supporting explanation.
It is helpful if the submission includes a brief introduction about you or your organisation. The submission should not have been previously published or circulated elsewhere.
For those wanting more guidance, a useful resource is that published by the UCL Public Policy team (it is about giving evidence to Select Committees, so needs to be read across, but it also includes a useful template).
